1. Complete and submit the On-Line Registration Form below to reserve your

classroom space.

2. Provide us with a copy of your dogs records for DHLP, PARVO, Rabies and

Bordetella prior to your first class.

3. Download and print the Registration & Waiver Form and bring a signed copy

of this form with you to your first class.
